My mother—stage IV metastatic breast cancer—was sent there for rehab after a severe UTI and hospital stay, in the hopes of returning home with a caregiver once regaining some strength. I acknowledge that my mother was very ill and had a lot going on—however the overall lack of concern for her wellbeing was concerning. She was struggling to eat, participate in therapy, and get out of bed. Nobody once brought these things up as a concern, and when I insisted on getting her transferred back to the ER for a higher level of care, I was shrugged off by her nurse. “We don’t usually transfer patients just because they’re confused” was her response. Luckily, another nurse took me seriously and spoke with her provider to get her transferred. She was on hospice the next morning, and she was gone leas than 48 hours later. No outreach from the facility, no concern by the staff. PLEASE do not send a loved one here.

Under federal nursing home regulations (42 CFR §483.15 & §483.10), facilities must provide: • A discharge summary • Medication instructions • Information necessary for safe transition of care Refusing that can be a violation. They refused my Nana all of the above. I’m still in contact with people about this place. It needs to be shut down. And I have on video them saying it’s not the law. I would trust these folks to take care of my FISH!

We are disappointed to hear about your Nana’s discharge experience. Please contact us at (864) 845-5177 to discuss these serious concerns immediately.

Piedmont primary care sucks they do nothing but send you around and around to specialists an the er when they can’t do the job. The pa tient is stuck with a big bill that nobody is could afford to pay. Would never recommend them ever. If you are a druggie you good to get treated. When the patient gets to er or specialists the doctors say why are u here , your primary care as should have handled this problem. We don’t see any reason for you to be here. And they say it’s not their job …

PIEDMONT POST-ACUTE, the building maybe older but the people who work there are so great to the folks there. Ive never seen anything like it. You’re greeted when you arrive. All day they have smiles on there face . They treated my mother whose been there twice in the last few months. When some leave going home for the day will come by and tell my mother bye and give her a hug. They treat them like family or friends . Most of these people this is there home and those that work there realize that and treat them with respect. Give them a chance , you wont regret it. Like all places there is room for improvement. The one big thing is waiting on someone to let you in or out. They should put a camera at the front entrance and a monitor at the nurses station with a keypad to open the door. Its an easy fix. Thank you all at PIEDMONT POST-ACUTE FOR ALL YOUVE DONE DOR MY MOTHER DURING HER STAY.

My mother was transferred from hospital to Piedmont on Wednesday for rehab. We signed her out on Thursday to take her to eat Thanksgiving dinner with us. Told them we would be bringing her back later. I was told to make sure I checked back in with nursing and signed her in when I came back. Had to be back by midnight. Arrive back just before 9. Locked up like Fort Knox. Ring the doorbell and wait multiple times. Pulled on the door so the alarm would sound a few times. Called multiple times. No one ever came out or answered the phone. About 39 degrees outside, so we were finally done waiting in the cold. Just turned around and brought her back home. Have called several more times and no answer or call back still. We are super irritated! Not the way things should be done. Also found out while she was with us, that she had multiple issues with staff not answering call lights for her to go to the bathroom and then when they finally did, it took them forever to come back and get her off. She could walk fine with her walker but wasn’t allowed to go on her own because therapy hadn’t evaluated her yet. On high doses of fluid pills for fluid retention and sitting and crying because no one will answer her to take her to the bathroom. Between the call light situation and not being able to get her back in the building we just kept her home and went to get her belongings. No one even called to see about her til shift change at 7:15 on Friday morning. We told the nurse we wanted to talk to DON/Admin and were told she would have them call us, never told us they weren’t back in the building until Monday. We called later in the afternoon and were told then they would be back on Monday. Just told them we’d come get her stuff. We were there within 15 minutes and they already had her stuff packed. No apology, no nothing. Didn’t try to talk us into bringing her back or anything. Between this experience and reading other reviews, please folks do your research before putting your loved ones here or anywhere else.

My father the only parent I have left is in this nursing home and for whatever reason I always have a hard time getting through I am disabled myself which is why he’s not with me I swear it’s like they cut the phones off on purpose. Not to mention like everyone else he’s also lost weight while being here y’all really need to do better. As for the response to meeting in person I don’t even live in the same state and the phone thing has been an issue for many people over a yr now as a nursing home that should be a top priority. Update still having problems getting in touch with my father I dont live in state all I wanted to do was wish him a happy Thanksgiving however the phone isnt being answered i myself am recovering from surgery and am trying to take the time out to reach him.

We do appreciate your concern. We are currently in the process of upgrading over our telephone service providers. If you have any questions or concerns about your father we are always happy to meet with you in person so that you have a full understanding of his condition and why physical changes may be occurring with him.

Families need to be aware of what happens inside Piedmont Post-Acute. My father was a resident here in September 2025 and experienced multiple preventable falls, medication transfer failures, and an unsafe discharge that left him at home without the correct seizure medications. I filed an official complaint with DHEC (Case # M10007-25). Despite a recorded conversation with a DHEC representative stating they “would find the facility at fault for the unsafe discharge,” I later received a form letter saying no violations were found. This contradicts the evidence I personally recorded duringmultiple interactions with both the facility and hospital staff. Piedmont Post-Acute has refused to provide the full case file after repeated written requests, which is a violation of 42 CFR § 483.10(g) (the federal right to access medical records). The public should also know there is at least one publicly filed lawsuit involving another resident who passed away under their care. Families deserve transparency so they can make informed decisions. Based on my family’s experience, I would strongly encourage anyone considering this facility to research thoroughly, ask directquestions, and insist on written documentation for all care-related decisions.
